New Patients

When making your first appointment, please have your insurance card and driver's license available for the receptionist to verify your insurance coverage and/or provide you with an estimated payment you will be responsible for at the time of service.  Please also remember:

 

• Arrive 15 minutes early to complete registration.  Print and fill out new patient forms ahead of time, click here.

• Bring a list of your medications, including dosage and how/when you take them, or put all of your medications in a bag and bring them with you.  If you need a Medical Record Form, click here.

 

All Patients

Inform the front desk staff of any change in name, address, telephone number, employment or insurance.  Inform your doctor of any changes in your medical history.  Notify us as soon as possible if you will be late for your appointment or if you will not be able to keep it.
